Intake

You will complete an application form that simply states that you are applying for DVR services, AND that you intend to go to work.

Your Counselor will ask you about your medical, financial, educational and vocational experiences. You will also review your skills, abilities and rehabilitation needs.

You MUST bring your IDENTIFICATION (see below) and your SOCIAL SECURITY CARD with you to your Intake Appointment with your counselor.

Colorado House Bill 1023 requires that you have one of the following pieces of identification when you apply for services:
Colorado Driver's License, or
Colorado Identification Card, or
Military Card or a Military Dependent's Identification Card, or
United States Coast Guard Merchant Mariner Card, or
A Native American Tribal Document
